Social Workers/ Social Work Coordinator

Key Responsibilities:

• Conduct assessments to identify the needs and challenges of patients/clients, and develop appropriate intervention plans.
• Provide support and guidance on healthcare options, financial assistance, and relevant services.
• Assist with healthcare discussions and ensure compliance with regulations and guidelines.
• Collaborate with healthcare providers, community organizations, and social service agencies to coordinate care.
• Maintain accurate records and track the progress of patients/clients.
• Provide administrative support for healthcare initiatives and programs.
• Coordinate patient referrals, follow-ups, screenings, and other necessary healthcare services.
• Assist patients with using healthcare-related applications and provide technical support when required.
• Support in tracking and reporting healthcare activities, including screenings and case management.
• Perform other administrative tasks as needed.

Requirements:

• Diploma or Degree in relevant fields such as Social Work, Nursing, Psychology, or Allied Health.
• Strong communication and interpersonal skills with a service-oriented mindset.
• Prior experience in customer service, healthcare, or social services is preferred.
• Ability to work independently and as part of a team in a fast-paced environment.
