Job Scope:

Making social visit and provide counselling to patients and their family
Assess financial and psycho-social status of patients and determine appropriate patient care plans
Coordinate and liaise with government party, healthcare professionals and other social service organizations
Participate in the implementation of support programs and improvement projects for the Medical Social Service Department
Maintain patient’s record and documentation

Job Requirements:

Degree in Social Work
Experienced candidate may be considered for Senior / Principal position
